About our services
The main role of clinical pharmacists is to work with other healthcare professionals to carry out reviews of medication by focusing on the following:
- preventing and correcting medication errors, thereby increasing patient safety
- promoting personalised dose adjustment
- increasing patients' adherence to and understanding of their medication
- improving the use of medicines for the elderly
- helping healthcare providers use medicines in a more cost-effective way.
Support for medication reviews
The doctor is always responsible for the prescription. The pharmacist's role is to review the medication and help provide the doctor with the best possible basis for decision-making in both simple and in-depth medication reviews.
When working with simple medication reviews, the pharmacist can help with the identification of the medicines a patient is taking and how these are used. During in-depth reviews of medication, the pharmacist systematically reviews all of a patient's medicines with regard to, among other things, whether the current indication exists, dosage in relation to morbidity and organ function, suitability based on treatment guidelines as well as side effects and interactions. It also looks at practical aspects such as dosing intervals, choice of dosage form and possible management problems. The pharmacist then provides feedback and any suggested changes for the doctor to consider.
Pharmacists in hospitals
Clinical pharmacists work at Uppsala University Hospital and Enköping Hospital and are available on weekdays from 8am -4pm. They can help with simple and in-depth medication reviews. They can also provide information to patients about, for example, newly prescribed medicines.
Pharmacists in primary care
Region Uppsala's primary care pharmacists are based at health centres, the elderly care unit and NÄVA. They are a resource for primary care physicians for the entire county, in both the public and private sectors, and support medication reviews for patients in both ordinary and specialised housing. The work can be done at the health centre, as a remote consultation or through team rounds, together with the doctor and nurse in charge.
Referral to a pharmacist
Doctors in public and private primary care and hospitals can use Cosmic's referral system to send a consultation referral to a clinical pharmacist for help with medication reviews or other medication issues concerning individual patients. As the receiving unit, select "Läkemedel Nära vård och hälsa" for primary care and "Läkemedel och farmaci" for hospitals.
